How We Support You – Simple Steps
Eligibility
Synerva Care Services Pty Ltd provides supports to participants who:
Are approved NDIS participants with funding relevant to the supports delivered
Reside within the organisation’s service area
Have needs aligned with the supports and services offered
Meet any specific program or service requirements
Initial Meeting (Intake and Assessment)
An initial meeting will be conducted to:
Identify the participant’s goals, needs, and preferences
Review the participant’s NDIS Plan and relevant documentation
Determine the suitability of services and supports
This process ensures supports are appropriate, person-centred, and aligned with the participant’s plan.
Service Agreement
Where services are agreed upon, a written Service Agreement will be developed outlining:
The supports to be delivered
Fees and funding arrangements
Roles and responsibilities of all parties
Terms and conditions of service delivery
The Service Agreement will be explained to the participant (and/or their representative) to ensure understanding. Services will commence once the agreement is signed by both parties.

Complex Support (Nurse-Led Care)
As a nurse-led provider, Synerva Health Group supports participants with higher and more complex needs. We work closely with participants, families, and healthcare professionals to ensure supports are delivered safely, with appropriate clinical oversight and coordination.

We provide nurse-led NDIS supports, with a focus on safety, dignity, and continuity of care. Led by experienced Clinical Nurse Managers, our team supports participants with both everyday needs and more complex care requirements, including high-intensity supports.
We aim to offer a reliable and responsive pathway for referrers, while ensuring participants receive care that is clinically informed and person-centred.
